We require a photo identification to be provided so that we can verify you and ensure that your full name, including middle names, are correctly represented on the certificates and statements we issue you. The certificate of completion will be used to lodge your application with ASMA and the ID you provide the post office with to complete this process must match the name on your certificate exactly.

Please upload your passport via a photo or image scan, alternatively a drivers license with your middle names spelt in full, or other AMSA photo ID.
